Each municipality's program for administration and enforcement of the State Uniform Fire Prevention and Building Code must include the features described in Part 1203 of Title 19 NYCRR. These forms are provided to assist municipalities and are only samples, intended to be used solely as a guide. Each municipality should consult with its own legal counsel in preparing its forms.

- In-Service Course Approval Form - Classroom: This form is for requesting a course to be approved as a classroom-based In-Service training course. Complete this form in its entirety and submit it to the DBSC. Incomplete forms will be returned and will delay the approval process.
- In-Service Course Approval Form - Online: This form is for requesting a course to be approved as an online In-Service training course. Complete this form in its entirety and submit it to the DBSC. Incomplete forms will be returned and will delay the approval process.
- In-Service Course Renewal Application: This application is for renewing a previously approved In-Service training course. Complete this form in its entirety and submit it to the DBSC. Incomplete forms will be returned and will delay the approval process.
- Professional Development Elective (PDE) Application:
This application is for applying professional development electives toward annual In-Service training. Up to 12 hours may be applied annually.
- BSI Certification Application:
Complete this application after you have successfully completed Basic Training Courses 9A, 9B, and 9C if you wish to become a certified BSI and do not wish to pursue a certification as a CEO.
- BSI or CEO Change in Level Certification Application: Complete this application if you wish to decrease your certification from a CEO to a BSI, or if you wish to increase your certification from a BSI to a CEO.
- Standard Instructor Certification Application:
This application is for applying to become a Standard Instructor. Standard Instructors are individuals certified by the Department of State to teach one or more basic training courses (19 NYCRR1208-1.2(s))
- Standard Instructor Renewal Form: This form is for Standard Instructors to renew their certification prior to its expiration date.
